Youth Player Registration Form & Medical Treatment Authorization Form (#R002-Y)

This form combines the player’s medical treatment authorization, and a confirmation the player is registering with your club. This form must be retained for at least five (5) years or until the player’s 18th birthday, whichever occurs last. This form is required and teams must certify they are in possession of it when cards are requested, the club will retain this form unless requested by US Club Soccer.

Standard registration fees will be refunded based on the following schedule:
Requests made before July 15: Full Refund
Requests made July 15-August 15: 50% Refund
Requests made after August 15: No Refund
All refund checks will be mailed the second week in September. The above refund schedule applies to any player who we are able to place on any team, regardless of whether it is with the team, coach, or teammates that were requested. Any player who we are not able to place on a team will automatically receive a full refund. All refund requests must be made by the player’s parent or guardian via e-mail to firstname.lastname@example.org
The Land Park Soccer Board approved for this season, a mandatory 5 volunteer hour requirement for each family and a $40 Volunteer Deposit. All families that complete their 5 hours will have their $40 dollar fee roll over to the next soccer season. At the end of the season, if your family has completed the 5 hour volunteer requirement, you will receive a code to use next season to waive the volunteer deposit. If a family does not complete their 5 hours of volunteer time, your $40 will be absorbed into the club and you will have to pay the volunteer deposit again next year. The Land Park Soccer Club is a non-profit organization run exclusively by volunteers. No board member receives any compensation for their time.
AFTER COMPLETING THE REGISTRATION
If you provided payment, a new picture of the player, and, if required, a copy of the player’s county issued birth certificate, during the online registration process, your registration is complete and you do not need to do anything else at this time.
Team Placement Notification
Coaches may begin to contact players as early as July 1st and must notify all players of their team placement by July 30th.
Practices begin around July 31st. Most teams will practice twice a week, on either Mondays/Wednesdays or Tuesdays/Thursdays. Practices will be held between 4:00 p.m. and 8:00 p.m. Younger teams will usually practice for an hour at a time, while older teams often practice longer. Your coach will inform you of your practice schedule when he or she calls to notify you about your team placement. We do not assign practice times until after teams are formed, so please note that we cannot accommodate requests for a particular practice schedule or location.
A Land Park Soccer uniform can be purchased at Kombat Soccer (5958 South Land Park Drive, Sacramento, CA) for $30 with tax. This cost includes a jersey, shorts, and socks. You can reuse the uniform multiple years. The uniform just needs to be acquired before the first game in early September.
In addition to a league uniform, all players will need soccer cleats, shin guards, a soccer ball, and a water bottle. When you purchase a ball, be sure to get the correct size. The appropriate ball sizes per age level are listed below.
· U6 and U8 players use a size 3.
· U10 and U12 players use a size 4.
· U14 and up players use a size 5.
